Design updates for the profile setup view

1. Need a better title than "Set up your profile". Maybe "Set your name and profile photo"?
2. This is mostly fixed because we automatically focus the text field and bring up the keyboard
3. The avatar icon thing should show whatever we're going to use for the user's avatar if they do not set a picture. If we do that, then I don't necessarily think we need to change the "pencil" icon for the "edit" button - the user will be literally editing what we will show to their friends.
